Admission required documents :

1. Identity card/MyKid/Passport
2. Medical card and Letter Of Guarantee from your health insurance company (where applicable)
3. Doctor’s referral note
4. Past or present medical records, x-ray films, or prior test results (if your physician has requested them)
5. A copy of advance order from the doctor.

Deposits

A deposit is required upon admission. The amount will depend on the nature of your treatment.
