Request for Medical Report
To request an official medical report from Kajang Plaza Medical Centre (KPMC Kajang), please review the required guidelines, application methods, and necessary documentation below.
Application Methods
You can submit your application for a medical report through either of the following channels:
- In-Person Application: Visit the Medical Records Department counter located at the main KPMC Kajang premises.
- Email Application: Submit your request form and scanned copies of the required documents to haidiliza@kpmc.com.my
Required Documentation
To safeguard patient privacy and strictly comply with medical confidentiality laws, the following identity documents must be provided at the time of application:
Request by the Patient
- Malaysian Citizens: A clear copy of your Identity Card (MyKad / MyKid).
- Non-Malaysian Citizens: A valid Passport.
Request by a Third Party (e.g., Family Member, Insurance Agent, or Lawyer)
- A formal Consent / Authorization Letter signed by the patient.
- A copy of the Patient’s Identity Card or Passport.
- A copy of the Applicant’s / Representative’s Identity Card or Passport.
- Proof of Relationship (e.g., Birth Certificate or Marriage Certificate) if the patient is a minor or incapacitated.
Processing Time & Charges
- Processing Period: Standard medical reports typically take 14 working days to complete from the date all correct documentation and payments are received. Complex specialist or insurance reports may require additional time depending on doctor availability.
- Fees: A standard administration fee applies depending on the type of medical report requested. Please verify the exact billing amount with our registration counters before payment.
